We are thrilled that you are ready to book your new vacation with us! Please review the following forms, waivers, and documents.

Depending on your situation, you may only need to complete certain forms, so please fill out the appropriate items below.

Each will have a dropdown box with a list of our advisors – please make sure to select the advisor that you are currently working with on every form. Thank you!

Form Type and Link	What it’s For
New Client Intake Form	For those new to MGA Travel, we need these basic details to start your reservation. You can use a single form for a single person or two people with the same address.
Service Agreement Form	If your advisor has advised you of the amount of your professional fee for your project, this is how you let them know you are ready to move forward.
Payment Authorization Form	This is where you securely provide the credit card information (or other payment type) that you would like us to pay for items you designate. These could include the deposit, insurance, professional fees, or general instructions. You’d also use this form to update us with new credit card information if things change.
Passport Upload Secure Form	If you are new to us, or you have recently received your Passport renewal, this is where you send it to us. A photo of the data page is required before we will work on any international airfare.
Drivers License Secure Upload	If your trip only requires domestic airfare, then you will only need to provide a photo of your current driver’s license. For domestic travel that includes May 7th, 2025 or later, you will need a REAL ID in order for us to book your domestic travel. Or you can travel with your passport, but the ticket needs to match the ID used.
Emergency Contact Form	This is someone we (or travel supplier) can contact in case of an emergency. Please ensure this is NOT someone traveling with you!
Minor Travel Form	Unless both custodial parents / legal guardians are traveling with a child under 18 years old, you will need to complete this form. This is especially true for divorced parents or grandparents traveling as supervisors and it includes medical authority.
Travel Insurance Form	Your advisor will quote a travel insurance policy during the booking process. This form tells us which policy you accept, or a waiver if you decline.
