We are thrilled that you are ready to book your new vacation with us! Please review the following forms, waivers, and documents. They have been linked, when possible, to our new software Tern.

Important: Each advisor has their own Forms page. Please ensure you are on the correct advisor’s page for your trip!

Depending on your situation, you may only need to complete certain forms, so please fill out the appropriate items below.

Each will have a dropdown box with a list of our advisors – please make sure to select the advisor that you are currently working with on every form. Thank you!

New Vacation RequestTell us what kind of trip you would like to take next! [Tern]
Client Profile FormFor those new to MGA Travel, we need these basic details to start your reservation. Please fill out a form for each guest.
This is also where you upload your Passport, whether you are new to us or you have been with us but have a renewed passport.
Existing clients may update their details (such as a new passport, etc) here at any time [Tern]
Client AgreementThis agreement must be signed for us to work together and you to acknowledge our Terms & Conditions. [Tern]
Service Agreement FormIf your advisor has advised you of the amount of your professional fee for your project, this is how you let them know you are ready to move forward. [Tern]
Payment Authorization FormThis is where you securely provide the credit card information (or other payment type) that you would like us to pay for items you designate. These could include the deposit, insurance, professional fees, or general instructions.

Note: As we migrate to our new software, the Payment Authorization will be sent to you within Tern, and this link will eventually be removed from this page.

You’d also use this form to update us with new credit card information if things change.

Roomate RequestWant to attend, but don’t have a friend to share a room? [Tern]
Drivers License UploadIf your trip only requires domestic airfare, then you will only need to provide a photo of your current driver’s license.

For domestic travel that includes May 7th, 2025 or later, you will need a REAL ID in order for us to book your domestic travel. Or you can travel with your passport, but the ticket needs to match the ID used. [Formsite Legacy]
Emergency Contact FormThis is someone we (or travel supplier) can contact in case of an emergency. Please ensure this is NOT someone traveling with you! [Tern]
Minor Travel FormUnless both custodial parents / legal guardians are traveling with a child under 18 years old, you will need to complete this form.
This is especially true for divorced parents or grandparents traveling as supervisors and it includes medical authority. [Tern]
Travel Insurance WaiverYour advisor will quote a travel insurance policy during the booking process. This form tells us which policy you accept, or a waiver if you decline. [Tern]
Cancellation Request FormNeed to cancel your upcoming cruise or trip? Please complete this form. Your advisor will let you know any cancellation costs and penalties. [Tern]
