Important Forms & Waivers

We are thrilled that you are ready to book a new vacation with us! Even if you are a longtime client of ours (thank you very much), you may need to update a credit card or upload your renewed passport. Any questions or concerns, please call 843-279-0310.

Depending on your situation, you will only need to complete certain forms, so please fill out the appropriate items below. Thank you!

We use Formsite to store and secure these items. Here is the security and encryption used by Formsite. Only our agents have access to the form data.

New Vacation Request	Tell us what kind of trip you would like to take next!
Client Profile Form	For those new to MGA Travel, we need these basic details to start your reservation. Please fill out a form for each guest. Existing clients may update their details (such as a new passport, etc) here at any time.
Client Agreement	This agreement must be signed for us to work together and you to acknowledge our Terms & Conditions.
Service Agreement Form	If your advisor has advised you of the amount of your professional fee for your project, this is how you let them know you are ready to move forward.
Payment Authorization Form	This is where you securely provide the credit card information (or other payment type) that you would like us to pay for items you designate. These could include the deposit, insurance, professional fees, or general instructions. You’d also use this form to update us with new credit card information if your card info changes.
Passport Upload Form	If you only need to provide or update your Passport, whether you are new to us or you have been with us but have a renewed passport, this is the form for you.
Outlander Trip Agreement	To be completed for Outlander Society adventures!
Drivers License Upload	If your trip only requires domestic airfare, then you will only need to provide a photo of your current driver’s license. For domestic travel that includes May 7th, 2025 or later, you will need a REAL ID in order for us to book your domestic travel.
Emergency Contact Form	This is someone we (or travel supplier) can contact in case of an emergency. Please ensure this is NOT someone traveling with you!
Minor Travel Form	Unless both custodial parents / legal guardians are traveling with a child under 18 years old, you will need to complete this form. This is especially true for divorced parents or grandparents traveling as supervisors and it includes medical authority.
Travel Insurance Waiver	Your advisor will quote a travel insurance policy during the booking process. This form tells us which policy you accept, or a waiver if you decline.
Transportation, Event, and Parking Waiver	This waiver is required in order to use our group transportation, park your vehicle, attend one of our events, or access a venue. (Examples: Ship Tour or Inspection, travel on a motorcoach, attend event, or parking for the same.).
Cancellation Request Form	Need to cancel your upcoming cruise or trip? Please complete this form. Your advisor will let you know any cancellation costs and penalties.
